  1. Visit nearest Anganwadi or Block Development Office — Officer assigned
  2. Submit application with birth certificate — Registered

  • Aadhaar Card — 12-digit UIDAI identification (Original or photocopy)
  • Domicile Proof — Proof of state residence (Voter ID/utility bill)
