  • Children under 2 years; also pregnant women

How to apply

Application mode: offline

  1. Visit nearest Anganwadi centre, PHC, or sub-centre — Vaccination schedule received
  2. Bring child for scheduled vaccinations — Free vaccinations given

  • Aadhaar Card — 12-digit UIDAI identification (Original or photocopy)
  • Birth Certificate — Issued by municipal corporation/panchayat (Original)
