How to apply

Application mode: offline

  1. Employer registers you under ESIS — ESIC IP number assigned
  2. Receive ESIC card — Card issued; usable at ESIC dispensaries/hospitals

  • Aadhaar Card — 12-digit UIDAI identification (Original or photocopy)
  • Bank Account Passbook — Active bank account (First page)
  • Passport-Size Photograph — Recent (JPEG/PNG)
